How about something about the size of a TV-B-Gone with a few IR LEDs and a sensor in front, a few visible LEDs (maybe a bar display?) and a button on top. Make them in pairs. "Fire" one at the other, if it "sees" the IR, its recorded as a hit on the visible LEDs on top. Five "hits" (or ten or whatever) ends the game. Maybe a reset button, or just a power switch. Really fancy might include a pager motor to vibrate and let you know you're "dead". Runs on button cells, or at most a few AAAs. Perhaps build them into altoids gum tins?
